US Compounding Factory (“US Compounding Factory,” “we,” “us,” or “our”) is committed to protecting the privacy of your protected health information (“PHI”). We are required by the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
1. How We May Use & Disclose Your PHI
We may use and disclose your PHI without your authorization for the following purposes:
- Treatment — to compound and dispense medications and to coordinate with your healthcare providers regarding your care.
- Payment — to bill and obtain payment for the products and services we provide, including verifying coverage and processing claims.
- Healthcare Operations — to support our pharmacy operations, including quality assurance, staff review, and business management.
2. Other Permitted or Required Disclosures
We may also use or disclose your PHI, as permitted or required by law, for purposes such as:
- Complying with federal, state, or local law.
- Public health activities, including reporting adverse events and product recalls.
- Health oversight, judicial, and law enforcement purposes.
- Averting a serious threat to health or safety, and other uses required or permitted by law.
3. Uses & Disclosures Requiring Your Authorization
Other uses and disclosures of your PHI not described in this notice will be made only with your written authorization. This includes most uses and disclosures for marketing purposes and any sale of PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
4. Your Health Information Rights
You have the following rights regarding your PHI:
- Access — to inspect and obtain a copy of your PHI that we maintain.
- Amendment — to request that we correct PHI you believe is inaccurate or incomplete.
- Accounting — to request a list of certain disclosures we have made of your PHI.
- Restrictions — to request limits on how we use or disclose your PHI, though we are not required to agree in all cases.
- Confidential communications — to request that we contact you in a specific way or at a specific location.
- Paper copy — to obtain a paper copy of this notice upon request.
- Breach notification — to be notified following a breach of your unsecured PHI.
5. Our Responsibilities
We are required by law to maintain the privacy and security of your PHI, to notify you in the event of a breach of unsecured PHI, to follow the terms of the notice currently in effect, and to obtain your written authorization for uses and disclosures not described here.
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
7. Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice will be posted on this page with an updated effective date.
